It's important to keep your contact information accurate. If somebody finds your pet, this is how we'll get in touch!

Licensing Address


Enter your pet's primary home address. It's important this address is accurate and always up to date in the event that your pet ever becomes lost.

Mailing Address


Your mailing address is where we send your tags and membership package. If this is different from your licensing address, uncheck the box and enter the address.

Your Pets' Information


Enter your pets' information here. Click "Add Pet" to add an additional pet; click "Remove Pet" if you've accidentally added one too many.

Give us a call

If you have any questions about licensing in Collier County or need a hand setting up your online account, give one of our helpful customer service representatives a call at 1-877-239-6072. Our customer service hours are 9:00 AM – 8:00 PM EST, Monday through Friday.

Our representatives will help complete your registration

If you do not currently have a valid rabies certificate on file, our representatives will walk you through entering the required information into our system through your online profile. You will be required to pay a fee for each of your pets' licenses as per the fee schedule below.

You will also be sent an email with a receipt. You can upload more information about your pet from your online account. If you do not have an email address you can license in person and your activation code will be mailed to you along with your tag. Please note that you will need a valid email to setup your online DocuPet profile.

Keep your pet safe

Once you receive your pet's tag in the mail in approximately two weeks time, make sure you attach it to their collar. It’s also important to ensure your pet's online profile is complete and up to date and that you're familiar with DocuPet's HomeSafe™ Service in the event that your pet ever goes missing.

Download or pick up a registration form

If you would like to mail your registration form or complete it prior to arriving at one of licensing locations, you can download and print our offline form. Note you must provide proof of current rabies vaccination in order to have your pet licensed.

If you cannot locate your certificates, please contact your vet for assistance.

Complete the registration form

First time licenses require you to provide basic details about your dog or cat as well as your contact information. This not only allows us to ship you your pet's tag, it’s also used to reunite you with your furry friend if they are ever lost.

You will be required to pay a fee for each of your pets' licenses. Please see the fee schedule below.

We only accept checks or credit card information by mail. If you are visiting one of our vendor locations you may pay by cash, check, debit or credit card. If mailing a check, please make checks payable to DocuPet.

After your registration and payment has been received and processed our records will be updated to reflect an active license status and your tags will be shipped within one business day from being entered in our system.

It is important to note that mailed applications are not always entered into our system immediately and this will often delay the shipment of your pet's tag. If you would like to ensure your license application is processed right away please license online, at Collier County Domestic Animal Services, or by phone.

You will be sent an email with a receipt and link to activate your online account where you can upload more information about your pet.
